Clomifene, a nonsteroidal SERM, blocks estrogen receptors in the hypothalamus, preventing estrogen’s negative feedback on gonadotropin release and boosting the hypothalamic–pituitary–gonadal axis.
Zuclomifene, a more potent isomer, maintains longer binding periods.
In a typical female hormonal cycle, high estrogen and progesterone from the corpus luteum inhibit GnRH, FSH, and LH around 7 days post-ovulation. Without fertilization, the corpus luteum breaks down due to insufficient beta-hCG, normally produced by the embryo to sustain progesterone and estrogen levels for pregnancy maintenance.
